Preventing cluster headache attacks with the vitamin D3 regimen

A daily vitamin protocol that has helped many patients reduce or stop their attacks

The vitamin D3 anti-inflammatory regimen is a preventive protocol for cluster headache built around high-dose vitamin D3 taken alongside a set of supporting nutrients. You take it every day to reduce the frequency of your attacks, and for some patients it stops a cycle entirely. It was developed by a cluster headache patient, Pete Batcheller, and the full protocol is documented at vitamindregimen.com.

Cluster headache attacks are thought to involve inflammation, and correcting a vitamin D deficiency seems to help many patients. It also turns out that low vitamin D is very common among cluster headache patients. One study showed that over 90% of patients were deficient.[1]

The evidence for the regimen comes primarily from patient surveys, but the effects reported are large and consistent across them. A survey of 110 cluster headache patients, published in the journal Neurology, found that 80% had significant reductions in their attacks and 60% stayed largely pain-free.[2] In a larger survey of 313 patients on the regimen, 82% had a 70%-or-greater drop in attacks and 53% became pain-free.[3] The regimen is now widely used in the cluster headache community.

Keep in mind that it usually takes a few weeks after the initial loading for the regimen to start working. Also, because the vitamin D3 doses involved are high, it needs blood testing and a doctor's oversight to do safely.

What the regimen involves

The protocol has two phases:

  1. A loading phase that raises your vitamin D level quickly, using 600,000 IU of vitamin D3 in total, spread over about one to two weeks. A single large oral dose on this order is known to raise blood vitamin D substantially within days.[4]
  2. A maintenance phase that keeps the level steady, usually at 10,000 IU per day (or 50,000 IU per week), adjusted by your doctor based on your blood results.

The regimen also involves taking additional supplements (referred to as "cofactors") alongside the vitamin D3. These help your body process the vitamin D3 adequately. They include magnesium, omega-3 fish oil, and vitamin K2.

Before you start, and again after the loading phase, you should get blood tests for vitamin D (measured as 25-hydroxyvitamin D), calcium, and parathyroid hormone (PTH). The target vitamin D level is 80 to 100 ng/mL (200 to 250 nmol/L).[5]

The vitamin D3 doses here are far higher than in an ordinary supplement. Do not start the regimen without a baseline blood test and a doctor who can monitor your calcium.
